Dear Dr. Dvorak,
Will you kindly send me a recent photograph of yourself - two different poses with your autograph on each - and, if you can write me some biographic[al] details, besides which is contained in the Musical Dictionaries. Also, loan me two manuscripts of about a page each with your autograph. You will greatly oblige us.
All the above is for publication and I would like you to write upon the two photographs and the two manuscripts “To The National Conservatory of Music of America - New York”
Kindly mail all before February 15th.
I enclose [a] list of books we use in the National Conservatory - of course if you wish any changes you are at liberty to do so. Do not worry about anything - the Americans will be very considerate.
